Reporting an Environmental Incident

To report a release of material that impacts, air, land, water (including sanitary and storm sewer), or other environmental noncompliance:

  1. Log in to the incident reporting portal.
  2. Select Report a New Incident.
  3. Select Environmental
  4. Review the instructions at the top of the Event Report tab. 
    1. Complete required fields and any additional information that is known.
    2. Select Save to move to the next tab.   
  5. Select the Env Release Details tab. 
    1. Select the + icon to identify a material that was released. 
    2. Provide all known information.
    3. Select Save and Close to move to the next tab.
  6. Select the Witnesses/Contacts Tab.
    1. Select the + icon to identify a witness, facility contact, or other contact related to the incident.
    2. Select Save and Close to move to the next tab. 
  7. Select the Documents tab.
    1. Select the + icon to attach a supporting document, like a photograph, floorplan, map, etc.
    2. Specify the Document Type, provide a description, and attach your document.
      • To submit notes, select General as the Document Type and enter your information in the Notes section.
    3. Select Save and Close to return to the tab view.
  8. Click the Submit button when finished.
  9. If the incident resulted in an injury, submit a separate Injury, Illness, or Exposure Incident report.
  10. If the incident involved a motor vehicle accident, submit a separate Motor Vehicle Accident report.
